Infant Discharged After Swallowing Magnet Without Symptoms
A one-year-and-four-month-old infant was recently taken to the emergency room at Meuhedet Health Services in Hadera after his parents noticed he had swallowed a magnetic ball. Upon arrival, the child was in good general condition with no outward symptoms, such as abdominal pain, coughing, or breathing difficulties. Medical examinations also revealed no unusual indicators.
Despite the child's stable condition, medical staff proceeded with an abdominal X-ray due to the suspicion of magnet ingestion. The imaging confirmed the presence of a circular foreign object in the upper abdomen, consistent with the parents' report. Dr. Muhammad Kaba, a pediatrician and medical director for Meuhedet's emergency departments in the northern region, emphasized that external appearance can be deceiving in such cases. He stated, "A child who has swallowed a magnet may appear well and feel fine even when the foreign body is still inside the digestive system, hence the need for examination and follow-up even in the absence of pain or other signs."
In this particular case, as the infant remained asymptomatic and in good general health, and given the location of the object on the X-ray, the medical team decided no immediate intervention was necessary. The child was discharged with instructions for home monitoring. However, the incident highlights the specific dangers of magnet ingestion in children, as complications can arise later even without initial symptoms.
Dr. Kaba explained the critical difference between swallowing one magnet versus multiple magnets. A single magnet might pass through the digestive system naturally. However, swallowing two or more magnets, or a magnet with another metallic object, can be far more complex. These magnets can attract each other across different parts of the intestines, potentially causing pressure on the intestinal wall, leading to damage, obstruction, fistulas, or even perforation over time. This damage may not initially present with clear symptoms, making the number of magnets a crucial factor in assessment. Even during imaging, multiple magnets stuck together might appear as a single object.
Parents were instructed to monitor the child for any changes and to ensure the foreign object passed. They were also advised to return for further evaluation if the magnet did not progress as expected or if new symptoms emerged, including abdominal pain, vomiting, bloating, fever, decreased appetite, blood in vomit or stool, or any general change in the child's condition. The primary lesson, according to Dr. Kaba, is that while not every ingestion requires immediate intervention, a medical evaluation is crucial, especially with magnets, to ensure safe passage through the digestive system and prevent complications. Prevention involves keeping small magnets and other small objects out of reach of young children.
