Israeli Medical Residents Earn Up to $25,000 Monthly, Driven by Overtime

Medical residents in Israel earn between NIS 15,000 and NIS 18,000 per month during their first year, with senior residents in their fourth or fifth year approaching NIS 25,000. The average salary for a resident position in government hospitals is NIS 23,905, according to state payroll data. This figure is significantly higher than the minimum wage of NIS 6,443.85 per month and the average Israeli salary of NIS 14,000-15,000. However, the difference lies in working hours, as residents work an average of 262 hours per month compared to the national average of 182 hours.

The base salary for residents is relatively low, calculated on a 45-hour work week for first-year residents and 42 hours after passing the initial stage exam, with hourly rates around NIS 28-31. The substantial increase in their paychecks comes from on-call shifts. A weekday night shift, from 4 PM to 8 AM, is compensated as four days' worth of pay, while an emergency room shift is equivalent to two and a half days' pay. Residents average 4.7 shifts per month in central Israel and five in the periphery.

The path to becoming a medical resident involves six years of university study, costing NIS 11,000-15,000 annually, followed by a one-year internship. After obtaining a medical license, specialization training begins, lasting four to six years depending on the field. Passing the Stage A and Stage B exams leads to becoming a specialist, with salaries significantly increasing. Junior specialists in government hospitals earn an average of NIS 34,418, senior specialists NIS 42,702, and department heads NIS 53,613.

Salaries vary by location and specialty. Residents in peripheral hospitals earn about 16% more than those in central Israel due to higher shift allowances and more frequent on-call duties. Choosing a specialty in high demand, such as anesthesiology or intensive care, can also qualify residents for significant bonuses. The choice of specialty also impacts long-term earning potential, with fields like geriatrics and ENT offering higher ceilings than pediatrics.

Compared to other countries, Israeli residents' salaries are comparable to those in Germany and the US when adjusted for working hours and cost of living. However, the Israeli system's high number of working hours leads to concerns about burnout and safety, with over half of residents reporting falling asleep at the wheel after shifts. Despite the demanding hours, residents benefit from full job security, comprehensive pension contributions, and a doubling of their salary within ten years.
