New Treatments Improve Quality of Life and Prevent Complications in Multiple Myeloma Patients

Multiple myeloma is a malignant bone marrow disease affecting plasma cells, leading to bone fractures, kidney damage, anemia, and elevated blood calcium levels. Historically, treatment relied on steroids and chemotherapy, which were highly toxic and less effective. Over the past decade, numerous biological drugs have been approved, significantly enhancing both survival and quality of life, including for elderly patients with comorbidities. Dr. Yulia Waksman, an internal medicine and hematology specialist managing myeloma and related plasma cell disorders, explains that treatment approaches have evolved from reserving innovative therapies for late-stage disease to incorporating them earlier to achieve deeper, longer-lasting responses and delay relapse.

Myeloma is a chronic condition characterized by periods of response and relapse, requiring sequential treatment planning. Current strategies emphasize personalized care considering patient age, comorbidities, preferences, mobility, and prior side effects, alongside the availability of therapies at different disease stages. The goal is to maximize benefit from each treatment phase and postpone disease progression, with decisions made collaboratively with patients aiming for prolonged life with optimal quality. Some novel treatments can induce very deep responses, sometimes undetectable by sensitive blood or bone marrow tests, raising cautious discussions about potential cures in select cases.

Despite advances, challenges remain in treatment sequencing, especially after first-line therapy failure. While first-line combinations offer excellent efficacy and safety, access to some innovative drugs at early relapse stages is limited. Consequently, some patients receive these treatments only after multiple relapses, when their health may be more compromised and options fewer. Ongoing efforts focus on improving availability and timing of therapies to enhance outcomes for all myeloma patients.
