Handgrip Strength Test Reveals Key Insights Into Aging and Longevity

In an era dominated by epigenetic watches, genetic tests, and artificial intelligence, a simple, inexpensive, and accessible measure, handgrip strength, has emerged as a crucial indicator of future health. Numerous recent studies show that weak handgrip strength reflects not only weak hands but also declines in muscle mass, nervous system function, metabolic health, and can predict early morbidity and mortality. Dr. Osnat Raziel, a specialist in obesity treatment, metabolic health, and healthy longevity at Assuta Ramat Hachayal’s multidisciplinary obesity center, explains that hand and forearm muscles are involved in nearly every daily activity, making grip strength a proxy for overall muscle and bodily health.

A large international study involving nearly 140,000 participants from 17 countries found that every 5-kilogram decrease in grip strength was linked to a 16% higher risk of all-cause mortality, a 17% increase in heart disease death, and a greater risk of stroke. Grip strength outperformed systolic blood pressure as a mortality predictor in this research. Beyond lifespan, low grip strength correlates with increased risk of falls and fractures, reduced daily functioning, sarcopenia (muscle loss), frailty, cognitive decline, and higher dementia risk.

Contrary to some social media claims, simply exercising hand muscles does not extend life. Grip strength mainly signals overall health, with stronger individuals typically being more active, having greater muscle mass, better aerobic fitness, and superior metabolic health. The best way to improve grip strength is through full-body muscle strengthening exercises, including weight carrying, deadlifts, pull-ups, rowing, and regular resistance training, alongside adequate protein intake, quality sleep, and aerobic activity.

Grip strength is most accurately measured using a handgrip dynamometer, which records force in kilograms. Longevity clinics use this measure to assess biological age, muscle mass, frailty risk, response to strength training, and monitor effects of weight-loss medications that may reduce muscle mass. A recent study published in JAMA Network Open involving over 5,400 women aged 63 to 99 found that higher muscle strength independently predicted longer survival, even after adjusting for aerobic fitness, physical activity, and other risk factors.

In summary, while not a magic test, handgrip strength offers a quick, simple window into muscle, nervous system, and overall health. When combined with other assessments like VO₂max, walking speed, chair rise tests, body composition, and metabolic evaluations, it provides a comprehensive picture of aging rate and the likelihood of maintaining a long, independent, and healthy life.
