Roche Launches Pioneering Trial to Prevent Alzheimer’s Before Symptoms Appear

Pharmaceutical company Roche has announced the launch of a large-scale clinical trial named PrevenTRON, aiming to prevent Alzheimer’s disease before any symptoms manifest. The trial, presented at the 2026 Alzheimer’s Association International Conference (AAIC) in London, targets individuals without cognitive decline but who show biological markers indicating a high risk of developing Alzheimer’s. Roche will identify participants using an advanced blood test called Elecsys pTau217, which detects early brain pathology related to the disease.

The experimental drug at the center of the trial is Trontinemab, a novel antibody developed with Roche’s Brainshuttle technology. This technology enhances the drug’s ability to cross the blood-brain barrier more efficiently, a major challenge in neurological treatment development. Early Phase I/II data are promising, showing that about 92% of patients receiving a high dose reached low brain amyloid levels within 28 weeks, a key Alzheimer’s marker. Additionally, the incidence of ARIA side effects, which are imaging changes linked to amyloid, was significantly lower compared to competing treatments.

PrevenTRON is part of a broader Roche program that includes Phase III trials named TRONTIER 1 and TRONTIER 2, which focus on patients in early Alzheimer’s stages. These trials are recruiting approximately 1,600 participants across 18 countries, with results expected around 2028. This initiative marks Roche’s return to Alzheimer’s research after previous setbacks with drugs like crenezumab and gantenerumab. While companies such as Biogen, Eisai, and Eli Lilly have led the field with amyloid-based treatments, Roche aims to compete with a faster, safer approach that intervenes before the disease’s onset.
