Early-Onset Type 1 Diabetes Linked to Higher Dementia Risk in Later Life
Recent decades have seen significant advances in managing type 1 diabetes, enabling patients to live longer and reach older ages. This shift has prompted researchers to explore the long-term effects of the disease, including its impact on brain health. A large-scale study published in Diabetes, Obesity and Metabolism, involving over 43,000 Swedish patients tracked for nearly 30 years, investigated whether the age at diabetes onset influences dementia risk later in life.
Participants were divided into three groups based on diagnosis age: before 10, between 10 and 17, and between 18 and 30. The study found that individuals diagnosed before age 10 had a higher risk of developing dementia compared to those diagnosed between 18 and 30. By age 75, about 7% of early-onset patients developed dementia versus 5% of those diagnosed later. Earlier diabetes onset also correlated with earlier dementia diagnosis, suggesting cumulative effects of prolonged disease exposure.
The strongest association was with vascular dementia, linked to impaired blood flow to the brain, with a 65% increased risk in the youngest diagnosis group compared to the oldest. No significant link was found with Alzheimer's disease, though diagnostic overlaps complicate interpretation. Researchers propose that early-onset diabetes leads to longer exposure to blood sugar fluctuations, chronic inflammation, and small blood vessel damage, potentially affecting brain aging and development.
These findings underscore the importance of early and optimal diabetes management to minimize blood sugar variability and control additional risk factors like hypertension, lipid disorders, and smoking, which can exacerbate vascular damage. Moreover, new immunological treatments such as Teplizumab, approved for use from age one, aim to delay clinical diabetes onset, potentially altering long-term outcomes.
While the study is observational and does not prove causation, it highlights the critical role of early diagnosis and intervention in type 1 diabetes to improve not only immediate health but also cognitive outcomes decades later. Prof. Tali Zuckerman-Yaffe of Sheba Medical Center and Tel Aviv University, a study co-author, emphasizes that modifying disease progression early may enhance brain health in aging populations.
