IDF Medical Experts Highlight Functional Benefits of Eyelid Surgery Beyond Aesthetics

Eyelid drooping, often seen as a natural sign of aging, can develop into a functional problem that impairs vision by narrowing the visual field. Dr. Shay Keren, an oculoplastic specialist at Hillel Yaffe Medical Center, explains that eyelid drooping encompasses three main conditions: ptosis (muscle weakness lifting the eyelid), excess skin on the eyelid, and eyebrow drooping, which is common in men and adds pressure on the eyelid area. While this condition is more prevalent after age 60, younger patients may also qualify for surgery due to genetics or other causes such as nerve issues or trauma.

Traditionally viewed as cosmetic, eyelid surgeries are increasingly sought for functional reasons, especially by men who often present later for treatment. According to Dr. Keren, 30%-40% of eyelid surgery patients in Israel are men, many unaware that the procedure can restore vision rather than just improve appearance. The decision for surgery is based on clinical evaluation measuring how much the eyelid obstructs the pupil’s center, with a key criterion being the upper eyelid margin positioned less than 2 mm above the pupil center.

The surgical approach depends on the underlying cause and may involve removing excess skin, strengthening the levator muscle, or lifting the eyebrow, though eyebrow lifts are not covered by public health insurance. The procedure typically lasts 40 minutes to an hour under local anesthesia, with incisions hidden in the natural eyelid crease. Recovery is generally quick, with swelling and bruising resolving within two weeks and minimal visible scarring.

Dr. Keren emphasizes the importance of early evaluation for those experiencing vision obstruction or needing to use forehead muscles to open their eyes. He encourages men not to hesitate or feel embarrassed about seeking assessment, as the surgery can significantly improve both function and appearance while maintaining a natural look.
