Israeli Toddler Diagnosed with Rocky Mountain Spotted Fever After Contact with Family Dog
A four-year-old girl identified as N arrived at Wolfson Medical Center with symptoms of fever, weakness, and rash. Although her parents did not notice any tick bites and she had not been in nature, blood tests raised suspicion of Rocky Mountain spotted fever, a potentially serious tick-borne illness caused by Rickettsia bacteria. Further investigation revealed that N had contact with the family dog, which could have carried ticks.
Dr. Diana Tsher, head of pediatrics and infectious disease specialist at Wolfson, explained that the disease often begins with symptoms resembling common viral infections, making early diagnosis challenging. The illness typically presents with high fever, headache, muscle pain, and later a rash that may involve the palms and soles. Sometimes a black scab appears at the bite site, but this is not always visible.
Diagnosis relies on specific blood tests detecting the pathogen, alongside nonspecific signs such as low white blood cells and platelet counts and liver enzyme abnormalities. Immediate antibiotic treatment is crucial to prevent disease progression, even before laboratory confirmation. N received appropriate antibiotics, improved, and was discharged home.
N's father expressed surprise at the diagnosis and emphasized the importance of raising awareness among parents and medical staff. Dr. Tsher advised that while most tick bites do not lead to illness, persistent fever with weakness and rash warrants medical evaluation, especially if there was contact with dogs or outdoor exposure. Preventive measures include tick control on pets and careful body checks after outdoor activities. Ticks can transmit infection year-round, though exposure is more common in summer.
